Job summary
A dedicated children's health organization in Wisconsin is seeking a Health Care Coordinator to manage access to essential services for children with complex needs. You will create and oversee individualized care plans, ensuring quality service delivery and effective communication between stakeholders. Requirements include a Bachelor's degree and 2+ years of relevant experience in health advocacy or clinical case management. This role is full-time and offers a fully remote work opportunity.
Qualifications
- 2+ years experience in health promotion, advocacy, or child welfare.
- Knowledge of Medicaid population and benefits preferred.
- Acceptable driving record and ability to be insured by CHHS.
Responsibilities
- Coordinate access to medical, dental, and behavioral health services for children.
- Develop and monitor comprehensive care plans for each child.
