Job Title: Wellbeing Support Practitioner (Days & Nights)
Service: Short Breaks Service
Salary: Up to £28,129.92 per annum
Location: Southwick
Position Type: Full-time, working up to 42 hours per week (24 hours guaranteed)
Working Pattern: Day shifts (07:45–20:00) or Night shifts (19:45–08:00), on a 4‑days‑one‑week / 3‑days‑the‑following‑week rotation, including alternate weekends
Expected Start Date: Mid-February 2026

We’re now expanding our services with the opening of our brand-new Short Breaks Service in Southwick , a much‑needed facility offering vital short break care for individuals and families across the local community. This is an exciting opportunity to join an expanding team at the very beginning of a new service that will make a lasting difference to people’s lives.

About the Role

We’re looking for compassionate, dedicated Wellbeing Support Practitioners to join our team across both day and waking night shifts . Whether you’re supporting individuals during the day with meaningful activities and community inclusion, or ensuring comfort, safety, and reassurance through the night — every shift will make a real impact.

You’ll provide high‑quality person‑centred care and support , tailored to each person’s unique needs. This includes personal care, medication administration, PEG feeding, emotional support, and helping individuals to engage in fulfilling daily experiences that promote independence and confidence.

Shift Patterns
  • Day Shifts: 07:45 – 20:00
  • Night Shifts: 19:45 – 08:00
  • Rota: 4 days one week, 3 days the following week (including alternate weekends)
Key Responsibilities
  • Deliver person‑centred support that reflects each individual’s goals, preferences, and routines.
  • Promote independence and build confidence through daily living skills and meaningful activities.
  • Support community engagement and social inclusion.
  • Provide emotional support and reassurance to individuals and families.
  • Administer medication and assist with personal care needs.
  • Maintain accurate records and handovers , ensuring clear communication between day and night teams.
  • Ensure a safe, calm, and supportive environment at all times.
  • Experience supporting adults with learning disabilities, autism, complex needs, or mental health conditions
  • Strong communication and interpersonal skills
  • Compassionate, patient, and person‑centred approach
  • Ability to work independently and as part of a team
  • Understanding of safeguarding and health & safety principles
  • Knowledge of personal care and medication administration
  • Full UK driving licence and access to a vehicle
  • NVQ Level 2 or 3 in Health and Social Care (or working towards)
  • First Aid certification
  • Experience supporting individuals in short break care settings (or similar)
