About you…

In our Adult Social Care Hospital Team,you will support our most vulnerable residents in getting their lives firmly on track, using a strength-based approach and always keeping the person at the heart of everything you do.

As a Social Worker, you will have the time to build meaningful relationships with the people you work with, making a real difference in their lives.

You will be part of a team that places a strong emphasis on integrated working with our partners, delivering holistic and seamless support to those who need it most.

We currently have vacancies for experienced and motivated Social Workers within the Adult Social Care Hospital Team who can respond to general enquiries and crisis situations.

About the team…

Our Adult Social Care service is composed of highly professional and supportive multidisciplinary teams. We have a “can do” and “no wrong door” ethos. Our teams work together as one service and go the extra mile to ensure our residents have the right support.

We work closely with health and other partners to ensure people avoid unnecessary hospital admissions. If hospital care is needed, we make sure it is only for as long as necessary and we focus on helping them return home as quickly and safely as possible.

We have a range of intermediate care and discharge to assess services that focus on keeping people in their own homes.

Recognising the challenges working in Adult Social Care, we make sure our staff are well supported. You will receive regular supervision and access to training. Our focus is on supporting staff to learn, grow and develop, helping them achieve their full potential. We actively listen to social care staff about the best ways to deliver services and encourage feedback and suggestions for continuous improvement.

  • You will manage a caseload, organising and prioritising the workload effectively, with regular supervision and guidance.
  • You will work as part of a multi-disciplinary team, connecting people and their families to their community and social networks enabling them to live within their own communities.
  • You will work collaboratively with multidisciplinary colleagues to ensure people have the right care, at the right time in the right place.
  • You will take a strengths-based approach to support people to live the best life they can and reduce dependence on long term care.
  • You will support the delivering and maintaining of high standards of performance across the service.
  • 28 days annual leave increasing to 33 days after 3 years’ continuous service, with the opportunity to buy up to 10 and sell up to 5 days.
