What you will do
  • Conduct comprehensive assessments of adult wellbeing and needs with an emphasis on early intervention and prevention.
  • Co‑produce detailed support plans with individuals, focusing on their strengths and desired outcomes.
  • Undertake mental capacity assessments, best interest assessments, and necessary applications to the Court of Protection.
  • Ensure safeguarding is central to your role, addressing any risk of abuse or neglect for vulnerable adults.
  • Perform risk assessments with a proportionate and least restrictive approach.
  • Collaborate with multiple professionals to coordinate support for adults with complex needs.
  • Manage a caseload efficiently, contributing to the duty rota, including occasional weekends.
  • Build a team culture focused on the voice of lived experience.
  • Engage in continuous learning and contribute to the team’s development by supporting colleagues and students.
  • Maintain accurate records and comply with departmental policies, ensuring consistent high‑quality service delivery.

Essential qualifications
  • Social Work degree CQSW DIPSW CSS
  • Registration with Social Work England
  • Full Driving Licence
