Job summary

A social services provider in the East Midlands is seeking a qualified Social Worker to join their Reablement Team. This role requires a Social Work degree and registration with Social Work England, alongside a minimum of 2 years of post-qualified experience. The successful candidate will work in a multi-agency setting to assist individuals recovering from injury or illness. We are looking for a Social Worker to join the Reablement Team.

About the team:

The reablement team works with individuals who has suffered an injury/illness to help regain their independence. You will work collaboratively within a multi - agency setting, the aim is to deliver short term goal - orientated interventions that promote recovery and reduce reliance on long term care services. This role is a short - term case holding opportunity.

About you:

To be qualified for this role you must hold a Social Work degree and be registered with Social Work England. You should have experience working within an adult short - term case holding team, with a solid understanding of relevant legislation.
