  • Contribute to the efficient management of patient flow across inpatient pathways, maintaining high quality care.
  • Mentor the team on navigating social care roles and the implications of The Care Act.
  • Support complex stroke case discharges with the Neuronavigator and other teams.
  • Deliver clinical expertise to external agencies as required.
