Job summary

Hours:Full Time (37 hours per week) - part time considered

We are seeking enthusiastic and well‑organised persons to join our Social Prescribing team, and have opportunities available within two of our services:

Our REACT team.

Providing social prescribing support for people referred through REACT (Reactive Emergency Assessment Community Team) based at Constantine House, Ipswich.

Our ‘Connect for Health’ team.

Providing social prescribing support for patients referred to us by Ipswich GP’s or other health care teams, based at our Tower House office (with the possibility of Ipswich outreach work in the future).

Social Prescribing offers support for the (non‑medical) socio‑economic issues that individuals face to help improve their overall health and wellbeing. Our social prescribing teams work with a variety of statutory organisations, health professionals and other VCSFE providers. They support referred individuals to identify areas where they need help and support and then facilitate or co‑ordinate the actions needed to resolve. This may include support that can aid independence at home, prevent hospitalisation, to make connections to services parents that can assist george? with money, employment, and housing or welfare benefits or to help someone reconnect with the community and reduce isolation.

To be successful in a social prescribing role, you will be personable, confident begun‑self‑motivation? self‑driven, and self‑motivated – ideally with recent relevant experience or able to undertake training to reach the required standard. You will be confident using a range of IT systems for case management purposes, have excellent communication skills and be able to self‑manage your workload and time. You will be able to manage client cases with attention to detail and possess excellent interpersonal skills working empathetically and professionally. You will have a passion for making a difference by providing a ‘person‑centred’ approach to the clients you work with.
