An exciting opportunity has arisen for an enthusiastic and motivated Social Care Practitioner to join the Erewash Short Term Assessment and Reablement Team.
The Erewash Short Term Assessment and Reablement Team provides a multi-disciplinary approach to assess, support, and enable adults to maximise their independence and assess for any potential long term care needs. We are committed to working in a person‑centred and strengths‑based way, recognising people's assets and strengths and enabling them to identify and reach their goals. The team is community based but will also support people leaving residential and nursing establishments and facilitate safe and timely discharges from some hospital settings. We are continuing to offer a hybrid approach to working, and our team is currently working from home and from the office based on the needs of the service.
The successful applicant will be able to demonstrate the necessary skills, knowledge and experience to undertake this role in order to provide a safe, accountable, cost effective and efficient service to our clients which helps them to achieve their outcomes.
