Qualifications

  • Available for part-time work (20 hours) or full-time (40 hours).
  • Experience supporting older adults in frailty or complex long-term conditions is vital.
  • A passion for proactive, frailty-focused care.

Responsibilities

  • Provide advanced clinical assessment and management for adults with complex conditions.
  • Lead and coordinate care pathways with various healthcare providers.
  • Deliver proactive clinical care including falls risk assessment and care planning.

Skills

Registered Nurse with valid NMC registration
Experience in community or primary care
Managing complex caseloads
