Qualifications

  • Significant post-registration experience in community or primary care, managing complex caseloads.
  • Experience of working with older adults, geriatrics, and complex long-term conditions.
  • Knowledge of current NHS policy and community nursing models.

Responsibilities

  • Provide advanced clinical assessment and management of elderly patients in their homes.
  • Coordinate care pathways with GPs and community teams to support complex needs.
  • Deliver proactive clinical care, including falls risk assessment and anticipatory care planning.
