This is an exciting opportunity for a qualified registered professional with at least 3 years’ case management experience to join us. Westcountry Case Management are a leading case management service supporting people in reaching their goals, maximising their potential and quality of life. We are long established, widely regarding, and highly trusted in the Westcountry and Wales regions. We operate in the independent sector, and the company is part of the Active Care Group.
You will retain a clinical case load as well as providing support to the Business Director and Clinical Lead with regards to clinical procedures and processes. In addition, you will take on supervisory responsibility for other case managers within the company and play a large part in inducting new case managers.
Westcountry Case Management provides case management to both adults and children and young people. We work with clients with catastrophic injuries, with a specialism in ABI. We also have clients with other complex conditions. We are registered with CQC & CIW to enable our clients to directly recruit their own support workers.
