About The Team

We are looking for Senior Social Workers across the following teams:

  • Older Peoples Team – OP Team West - Harpenden & Villages Locality Team (Apsley, Hemel Hempstead) 37 hours per week
  • Connected Lives Gateway Team – Gateway North, Farnham House, Stevenage – 37 hours per week
  • Adult Disability Team – East - Autism specialist - fixed term/secondment

The Older Person Service is a dynamic service that works across the community under the Care Act to safeguard, assess, plan support and review adults and cares with long term health and social care needs. We offer hybrid working arrangements, flexible working practices, community engagement and volunteer activities, and support your continuous Learning & Development.

Role Overview

Be part of the front door to adult social care in Hertfordshire, focusing on prevention, early intervention, and short-term support. This role involves traveling throughout Hertfordshire to visit residents' homes and the office, providing crucial information and support to prevent, reduce, and delay the need for statutory services. You will work within a 12-week pathway to connect people with resources that empower them to manage their lives independently. Specialist training will be provided to assess functional ability and utilise technology and equipment-based solutions.

About The Role – Responsibilities
  • Coordinating different social workers and support services
  • Managing schedules and handling issues or concerns within the community
  • Providing emotional support and guidance to individuals and families
  • Assessing and reviewing the situations of individuals and families
  • Organising and managing packages of support to enable people to lead fuller lives
  • Making referrals to other agencies and participating in multidisciplinary teams
  • Maintaining accurate records and preparing reports for legal action
About You – Qualifications

You will be a SWE registered Social Worker, at least 2 years post qualified, who has successfully completed an ASYE program working to the Social Work England professional standards. You will undertake Care Act assessments for eligible current and historical cases, using our Connected Lives strategy to review and manage packages of care. You may also actively undertake a specialist role (e.g., AMHP, Best Interest Assessor or Practice Educator) or have a place secured on a course, or commit to undertaking this as soon as appointed.

This is a fast-paced, community-focused environment. A full UK driving licence is required. Hybrid working is available, with a minimum of 2 days per week in the office.

  • Secondment information: The Autism position is open to secondments. If you work for a Local Authority and wish to be considered as a Secondment, please complete the Secondment Form; otherwise the role is Fixed Term.
