Hours: 30 hours, Monday-Friday

About AMS Case Management and the Role

We are a growing and dedicated case management company based in Bolton, Greater Manchester. We provide services to clients across the North West and North East of England

For 10 years, we have built a reputation for excellence in rehabilitation, valued not only by our clients but also by our employees.

Case management is the management of a multi-disciplined team of clinical specialists to assess, plan, and monitor the needs of an individual and the services they require to enable the individual to meet their rehabilitation goals, returning them as close as possible to their life before their accident.

Some of our clients directly employ their own support workers as part of their rehabilitation. We require a Rehabilitation Support Coordinator to lead, coordinate, and facilitate the support provided to our clients, including the management of their support team, alongside the case manager, in line with CQC and IRCM guidelines.

Pay is negotiable depending on experience, plus company pension, mileage, and client expenses.

The role is office based, although travel to clients and support workers will be required.

Hours will be Monday to Friday, 30 hours per week.

Job Description

To ensure our client receives the highest quality of support in line with the values of AMS Case Management, the Rehabilitation Support Coordinator will be employed:

  • To devise, implement and review person‑centred support plans for the client, including necessary risk assessments, risk management plans, goal setting and support guidelines to ensure the service is continuing to meet client needs and wishes, alongside the case manager and clinical team.
  • To manage all aspects of the client’s support team and provide clear leadership to ensure staff are clear about their respective duties and responsibilities and have the support they need to carry out their roles safely.
  • To work alongside the legal team and case manager to safely recruit, onboard, induct, train and develop members of the directly employed support team, based on required skill sets, availability, and client preferences.
  • To ensure all support workers are trained in line with CQC requirements, refresher training is completed as required and that any specific client related training is completed.
  • To develop and maintain support staff schedules to ensure timely and appropriate coverage for all client needs.
  • To monitor and coordinate support worker annual leave and sickness and submit monthly timesheets for all of the support workers to the Payroll Company.
  • To lead regular team meetings, supervisions, annual appraisals, and the overall monitoring of staff performance, including competency checks for the support workers.
  • Complete monthly audits of client and support worker documents, relating to the client’s support.
  • To effectively record and investigate complaints, concerns, and incidents.
  • To ensuring the adherence and implementation of AMS Case Management policies and procedures, as well as external policies relevant to the organisation, in line with compliance requirements.
  • Promote the human rights of each client, keeping them at the centre of all decisions regarding them and their support.
  • Ensure that all information about clients and staff is kept secure and confidential.
  • To ensure any concerns regarding the client, and/or any safeguarding concerns, are reported to the Director and the Designated Safeguarding Lead, to ensure reports and concerns around safeguarding are reported to the necessary external agencies as per the Safeguarding Policy and investigated in a timely fashion.
  • To make the necessary Statutory Notifications to CQC as and when required.
  • Maintain full and accurate records and reporting systems in accordance with legal requirements and to ensure the effective running of the service.
  • Be responsible for the safe delivery of the service in line with CQC regulatory requirements, legislation, policies and procedures and best practice.
Experience Required

Experience managing a service and/or team regulated by the CQC.

Experience recruiting and inducting new staff.

Experience in delivering person‑centred support planning.

Experience of providing person‑centred support to individuals with catastrophic injuries including but not exhaustive, Acquired Brain Injuries, Spinal Cord Injuries and amputations.

Experience of working positively with families and health professionals.

Qualifications Required

A Level 5 in Leadership and Management in Adult Social Care.

Mandatory training as part of the commitment towards own professional and personal development.

Additional Requirements / Information

A knowledge and understanding of the case management role within catastrophic injuries would be beneficial.
