The ISW role is an opportunity to provide outstanding care and support to patients in their own homes. The role involves working alongside Senior ISWs, Nurses and Therapists to optimise patient outcomes. ISWs provide personal care, meds support and support with eating and drinking as well as working towards further competencies to further support the Multidisciplinary team.
The Urgent Community Response teamwork alongside the integrated night service who provide cover overnight
