Job Description

Purpose of the post: Ensure that the people of Gwynedd are at the heart of everything we do; help individuals to live their lives as they wish; take on the full care role in the home; collaborate with other agencies, attend reviews, meetings with health, consulting with specialists; administer medicines.

Responsibilities
  • Ensure appropriate use of users' property e.g., medicine and cash.
  • Make appropriate use of specialist equipment to assist the individual to live his/her life as they wish; report deficiencies and follow practice protocols.
  • Support the individuals who use the service – full charge of home role if appropriate; take responsibility for the residential care home and responsible person role.
  • Lead staff with their work duties; take decisions in the absence of managers; lead in emergencies; respond to enquiries.
  • Identify changes in health and well‑being of individuals and respond quickly and appropriately.
  • Be progressive and willing to make informed decisions independently.
  • Build relationships with people to understand what would make a difference to them and empower their voice.
  • Be innovative and creative in care; participate in activities and interventions; encourage service users to develop social skills and problem‑solving abilities.
  • Contribute to multidisciplinary talks about personal well‑being outcomes and ways to support individuals.
  • Maintain administrative work in accordance with guidelines; support with housekeeping and meal preparation.
  • Operate and maintain IT systems.
Training and Supervision
  • Self‑development and commitment to follow training programme in line with National Requirements.
  • Attend and contribute to supervision and team meetings.
  • Register with Care Council for Wales and abide by professional code of practice.
Safeguarding
  • Responsible for reporting any concern or suspicion that a child or vulnerable adult is being abused.

  • Manage information in accordance with Council’s information management standards and Data Protection.
  • Ensure compliance with Health and Safety regulations and Council’s Health and Safety Policy.
  • Operate within the Council’s equal opportunities and equality policies.

Special Circumstances
  • DBS/Disclosure and Barring Service processes required.
  • Work unsociable hours, rota basis, including bank holidays and weekends.
  • Work across other service sites if necessary.
