Overview

Job Type: 12-month fixed term contract ends 14 March 2027.

Felixstowe Integrated Neighbourhood Team (INT) consists of district & community nurses, generic workers, community matron, occupational therapists, physiotherapists and Social Care Workers. The team is governed and supported by a Lead Therapist, Lead Nurse, Lead Social Worker and the INT manager. The team is dynamic and works with close relationships across NHS developments, including GP practices, acute hospital colleagues, GP Federations, Social Prescribers, SuffolkMind, NSFT and local community groups. You will be supported by regular supervision and annual appraisal, work autonomously on behalf of local patients, and have access to skills development training relevant to the post. Our service runs core hours 08:00-18:00, 7 days a week. Staff are expected to work approximately 1 in 10 weekends, with flexible working available within service provision.

Main duties of the job

You will provide therapy interventions to residents of Felixstowe and surrounding areas aged 18+ registered with a Felixstowe GP. Our aim is to support, maintain and encourage healthy lifestyle choices and behaviours in ageing and living well processes. Conditions include long-term neurological conditions, acute medical episodes, falls and frailty, palliative and end of life care; all with a view to preventing admission and supporting early discharge. Many patients have complex needs across physical, mental and social care.

Essential
