Qualifications

• Experience working within Adult Social Care and/or Learning Disability services.
• Ability to work confidently in complex, fast-paced, multidisciplinary environments.

Responsibilities

• Undertake assessments of individual and carer needs and risks.
• Work collaboratively with service users and carers using a person-centred approach.
• Apply core social work principles and values.
• Maintain high standards of service provision.
• Make sound professional judgements in complex situations.
• Work in partnership to ensure holistic support plans.
• Maintain accurate case records.

Skills

Good working knowledge of Care Act 2014

Good working knowledge of Mental Capacity Act 2005

Good working knowledge of Safeguarding Adults procedures

Ability to work confidently in complex environments

Education

Degree or equivalent qualification in Social Work

Registered with Social Work England
