Qualifications

• Experience of working with service users and carers in successful delivery of treatment plans.
• Complete preceptorship programme for newly qualified nurses.
• Proven ability to develop therapeutic partnership with service users and carers.

Responsibilities

• Work as part of a team under the crisis model offering a 24/7 service.
• Deliver high quality care for patients at home.
• Participate in multi-disciplinary teamwork for treatment plans.
