Hywel Dda Health Board are currently looking to recruit Community Mental Health Nurses to join the Community Mental Health Centre based at Haven Way in Pembroke Dock, Pembrokeshire.

Our services work through an assessment & recovery approach with people accessing secondary mental health services for severe and enduring mental illness.
The post holder will be responsible for the assessment of care needs, the development of programmes of care and the implementation and evaluation of these programmes of care for a defined clinical caseload including the role of a Care Co-ordinator within the Care Treatment Planning under the Mental Health (Wales) Measure 2010.
The post holder is required to work without direct supervision for their delegated caseload. The post holder will have effective communication skills and develop positive working relationships between all healthcare professionals, statutory and voluntary agencies. The role with cover 5 days per week.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.
