About the Role

Newport Pagnell Medical Centre is looking to recruit a community matron nurse in their community health and support team.

The purpose of the role is to deliver proactive, holistic care for patients with complex health and social needs, supporting the community matron and Integrated Community Support Team (ICST) functions. The role aims to improve health and wellbeing, prevent unnecessary hospital admissions, and promote independence. The post holder will also deputise for the senior nurse when required.

Key Responsibilities
  • Receive and triage referrals for patients with complex health and social care needs.
  • Undertake advanced clinical assessments.
  • Develop and review personalised care plans, promoting self care and independence.
  • Provide medication support and education; assist with prescribing if qualified.
  • Collaborate with GPs, social care, frailty teams, district nurses and voluntary agencies.
  • Attend multidisciplinary team meetings and contribute to integrated care planning.
  • Support dementia care, memory assessments, annual reviews for housebound patients and bereavement support.
  • Assist in patient support groups and carer clinics.
  • Maintain accurate and detailed patient records on electronic systems.
  • Deputise for Senior Nurse, supervising HCAs and Care Co-ordinator when required.
  • Ensure compliance with clinical governance, safeguarding and quality standards.
About the Candidate

All applicants should have the following qualifications, registrations, skills and experience:

  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Evidence of continuing professional development.
  • Advanced clinical assessment skills.
  • Mentorship or teaching qualification.
  • Significant experience in community nursing and managing long term conditions.
  • Experience in case management and holistic assessment.
  • Ability to work autonomously and as part of a team.
  • Evidence of collaborative working across health and social care boundaries.
