Newport Pagnell Medical Centre is looking to recruit a community matron nurse in their community health and support team.
The purpose of the role is to deliver proactive, holistic care for patients with complex health and social needs, supporting the community matron and Integrated Community Support Team (ICST) functions. The role aims to improve health and wellbeing, prevent unnecessary hospital admissions, and promote independence. The post holder will also deputise for the senior nurse when required.

Newport Pagnell Medical Centre is based in the town of Newport Pagnell and we have further sites within the Kingfisher area and Willen areas.
We are a large training practice working across the three sites and we have our own PCN - The Bridge.
