Job description
Community Matron – Rosedean House Surgery

Location: Community-based – Rosedean House Surgery & patient homes
Flexible working: Full-time (40 hours) and Part-time can be considered

Rosedean House Surgery is seeking an experienced Community Matron to join our primary care team.

The Community Matron will support patients of all ages with diverse and often complex health needs across the Liskeard community. They will provide personalised, holistic care for adults with long-term conditions, frailty, and multiple comorbidities — helping patients live well at home, in care homes, or in other community settings. Given that a significant proportion of our population is older, this role will include a focus on supporting frail and elderly patients, while also addressing the broader needs of adults of working age with complex or chronic conditions.

The Community Matron will play a central role in:
  • Enabling patients to remain independent and at home for as long as possible
  • Reducing avoidable hospital admissions
  • Coordinating care across primary, community, and social care services

This position is ideal for a clinician who is passionate about proactive, patient‑centred care, enjoys working across a diverse community, and wants to make a meaningful difference in the lives of both older adults and younger adults living with complex health needs.

Key Responsibilities
Clinical Care
  • Provide advanced clinical assessment, diagnosis, and management of adults with complex, long‑term conditions — including frail, elderly, and housebound patients — in their homes, care homes, and community healthcare settings.
  • Deliver proactive clinical care, including falls risk assessment, polypharmacy review, and anticipatory care planning.
  • Work closely with families, carers, and social care teams to support patients with complex needs.
Leadership & Coordination
  • Lead and coordinate clinical care pathways with GPs, community nursing, care homes, therapy teams, social care, and voluntary sector partners.
  • Support the development of proactive care initiatives and early intervention within the practice and PCN.

About You – Ideal Candidate

You will be someone who enjoys building strong, trusting relationships with patients and their families. Experience in supporting older adults — including frailty, geriatrics, care homes, or complex long‑term conditions — is highly valued, while also recognising the broader needs of adults of working age.

Essential Qualifications and Experience
  • Registered Nurse with valid NMC registration
  • Independent Nurse Prescriber (V300)
  • Significant post‑registration experience in community or primary care, managing complex caseloads
  • Experience of multi‑agency, multidisciplinary working
  • Knowledge of current NHS policy, community nursing models, and primary care priorities
