Job description

We have a vacancy for a Clinical Pharmacist to support a number of services being delivered across our Frailty Service based at the Jean Bishop Integrated Care Centre in Hull.

Role Overview

Our Clinical Pharmacists work with a team of doctors, nurses and therapists providing a comprehensive geriatric assessment to patients seen in the Jean Bishop Integrated Care Centre, care homes, community bedded units and our frailty hospital at home (virtual ward).

Key Responsibilities
  • Deliver structured medication reviews as part of the frailty team.
  • Collaborate with multidisciplinary team to provide patient‑centred care.
  • Support current services and projects within the Frailty team, including flexible working arrangements.

  • Interest in frailty and patient‑centred care.
  • Independent means of transport to travel across the geographic area of services.
Working Hours

Part‑time hours (2 and 3 days) with flexible working to support services.
