Experienced Care Coordinator (min 6 months recent Care Coordinator experience is essential)

Location: 23 High St, Chipping Sodbury, Bristol BS37 6BA

£27,000 - £30,000 depending on experience | 8.30AM – 17.00PM Monday to Friday

Responsibilities

As a Care Coordinator, your key responsibilities will include:

  • Allocate appropriate care assistants to customers based on individual needs and availability
  • Coordinate care delivery to ensure services are provided within agreed timescales
  • Ensure all customers receive a high-quality, person-centred service
  • Maintain accurate and up-to-date customer records and care schedules
  • Work closely with colleagues to support effective team communication and coordination
  • Support the smooth running of daily operations to ensure continuity of care
  • Assist with resolving scheduling or service issues to maintain outstanding customer service Being the first point of contact for customers and care assistants
  • Creating and maintaining rotas using One Touch rostering system making coordinating calls easy
  • £27,000 - £30,000 per year (depending on experience)

  • Work as part of a team of 3 Coordinators, and within a large and supportive office team.

Requirements
  • At least 6 months recent experience of Care Coordinating essential

  • Have excellent communication, administration and organisational skills
  • Ability to grow your area
  • Qualification in Health and Social Care preferred but not essential.
  • Be part of the on‑call and back‑up rota, delivering hands‑on care when required
