Role Overview

A specialist mental health provider is seeking a committed and experienced Social Worker to join a multidisciplinary team supporting adults with complex mental health needs within a secure and rehabilitation setting.

This role offers the opportunity to make a genuine impact on the lives of vulnerable individuals by delivering high-quality social care interventions, safeguarding leadership, and discharge planning within a structured and supportive clinical environment.

Key Responsibilities
  • Manage a complex caseload of adults with mental health and associated social care needs
  • Complete social care assessments, risk assessments and care planning
  • Contribute to Care Programme Approach (CPA) and Integrated Care Review (ICR) meetings
  • Lead on safeguarding processes, including referrals to LADO and Local Authorities
  • Educate and support hospital staff on safeguarding and statutory responsibilities
  • Undertake annual social care reassessments in line with CPA frameworks
  • Facilitate access to welfare rights, housing, advocacy and community support services
  • Support discharge planning and liaise with home authorities and external agencies
  • Manage internal safeguarding investigations in line with policy and legislation
  • Participate in regular supervision and professional development
Person Specification
  • Degree in Social Work (or equivalent recognised qualification)
  • Registration with Social Work England
  • Experience working in a mental health, forensic, secure or similar complex care environment
  • Experience supporting vulnerable adults
  • Strong understanding of safeguarding legislation and statutory responsibilities
  • Excellent communication, report writing and multidisciplinary working skills
  • Ability to manage complex casework and competing priorities

You will be based within a specialist mental health service that includes medium secure, low secure and rehabilitation units. The service provides a seamless internal care pathway, supporting individuals with complex mental health needs, personality disorders and histories of offending or placement breakdowns.

The multidisciplinary team delivers personalised assessment and treatment programmes, focusing on recovery, rehabilitation and long-term independence.
