What You Will Do
  • Undertake assessments of service users and carer needs using a person‑centred approach in accordance with statutory and service procedures.
  • Utilise management information systems to analyse and update information, ensuring compliance with procedures and data protection legislation.
  • Devise and implement individual support plans, securing appropriate services and interventions based on assessed needs and risks.
  • Monitor and review plans relating to the assessed needs of users, families, and carers.
  • Investigate and manage risks using a Safeguarding framework when individuals face potential harm.
  • Provide concise written and verbal reports for potential legal contexts.
  • Identify and implement responses to concerns about practice or procedures, seeking guidance if required.
  • Convene and chair meetings, developing relationships that promote integrated and holistic practices.
  • Work to improve outcomes for individuals from diverse communities.
