Job Title: Locum Social Worker – Adults Hospital Team
Location: Huddersfield Royal Infirmary, Huddersfield, UK
Hourly Rate: £35.00
Contract Type: Full-Time, Ongoing (37 hours per week, Monday to Friday)

Step into a rewarding role as a Social Worker within the Adults Hospital Team at Huddersfield Royal Infirmary. You will play a key role in hospital discharge planning, working in a supportive team committed to safe transitions from hospital to home or community settings.

Responsibilities
  • Undertake Care Act assessments and develop tailored care and support plans to facilitate safe and timely hospital discharges.
  • Complete Mental Capacity Act assessments and participate in Best Interest decisions as required.
  • Attend and contribute to post-discharge reviews, including Decision Support Tool (DST) meetings where appropriate.
  • Address safeguarding concerns and contribute to adult protection work in line with statutory requirements.
  • Join the team duty rota (after initial familiarisation period), assisting with the management of incoming cases and maintaining efficient team operations.
