A local government body in Greater London is seeking a Social Care Assessor for its Hospital Discharge Team. In this full-time role, you will help support vulnerable adults through assessments and Care and Support Plans, ensuring safe and timely discharges. Ideal candidates will have strong communication skills and some experience in social care.
