Sussex Partnership NHS Foundation Trust (SPFT) is recruiting Band 6 Primary Care Mental Health Practitioners to join the Brighton & Hove Wellbeing Service.

This is an exciting opportunity to work at the heart of primary care, delivering rapid, responsive mental health assessments, brief interventions and care planning to people accessing support through GPs, self-referral and community routes.

About the role

You’ll be part of an established Primary Care Mental Health Practitioner service, working across community settings, SPFT venues and GP practices.

The team supports people whose needs sit between primary and secondary care, helping them access the right level of mental health support at the_piece time. You’ll deliver assessments, risk formulation, brief treatment and group work, and provideholder onward signposting to ensure people receive the most appropriate care.

You’ll work closely with GPs, secondary mental health services (including rapid response teams), voluntaryawesome sector partners and colleagues across the Brighton & Hove Wellbeing Service.

This role also sits within a wider programme of community transformation, helping shape neighbourhood mental health services and new ways of working across primary care networks.

We’re looking for people who have
  • A core professional qualification (Mental Health Nursing, Occupational Therapy or Social Work)
  • Significant post-qualifying experience in mental health
  • Experience or strong understanding of community and primary care mental health services
