Job Summary

Band 5 Physiotherapist to support the Home First community service based in Leek, operating across North Staffordshire and Stoke-on-Trent. The role provides short-term reablement to enable patients to maximise independence and manage health and social care needs, and facilitates safe hospital discharges.

Key Responsibilities
  • Deliver autonomous physiotherapy assessments and treatments within a community setting, managing a caseload in collaboration with multidisciplinary teams.
  • Address a range of care/support issues across the spectrum of conditions, aiming to reduce hospital admissions and improve health and wellbeing.
  • Apply professional knowledge and judgment to select appropriate care/support packages, considering individual circumstances and preferences.
  • Engage with individuals and carers who may present challenging behaviour, applying management strategies and maintaining therapeutic relationships.
  • Travel independently across Leek Moorlands, North Staffordshire, and Stoke-on-Trent to support patients’ needs.
  • Work within the service’s operational hours (09:00‑18:00) and cover required shift patterns (5 days over a 7‑day week).
Qualifications
  • Degree in Physiotherapy.
  • Registered with the HCPC.
Desirable
  • Willingness to undertake further training relevant to the post.
  • Evidence of further clinical study.
  • Experience working within multidisciplinary community teams.
Essential Skills, Knowledge & Abilities
  • Good working knowledge of Physiotherapy Care Pathways and their objectives.
  • Experience managing frail elderly patients, including assessment, diagnosis and treatment.
  • Ability to organise and prioritise caseloads effectively.
  • Commitment to client‑centred, non‑discriminatory practice.
  • Ability to apply detailed clinical competence within the Discharge to Access pathway.
  • Flexibility to work 7‑day services and two shift patterns, with readiness to travel within a geographical area.
  • Capacity to make judgements, decisions and perform well under pressure.
