Position Summary

The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of stepdown services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to eligibility, into Midas for trending report purposes.

Job Duties/Responsibilities
  • The Step-Down Navigator will collaborate with the following departments:
    • Clinical Services: To ensure the patient is clinically appropriate to participate in programming
    • Business Office: To ensure the patient has covered benefits and to discuss copay responsibilities
    • Inpatient Utilization Review: To ensure the patient is authorized to step-down to a different level of care
    • Outpatient Services: To ensure the outpatient program is aware of the patient’s anticipated admission date
