Qualifications

• Good understanding and experience supporting people with a learning disability and/or neurodiversity.

Responsibilities

• Collaborate with stakeholders and partner organisations.
• Provide tailored tier 2 weight management and stop smoking support.
• Record and report activity on the patient database.
• Support the development of resources and review service aspects for inclusivity.
