Qualifications

  • Must have the ability to travel locally for home visits, which includes having access to own vehicle with business insurance.
  • Flexibility to cover annual leave and sickness when required.
  • Evidence of continued professional development.

Responsibilities

  • Deliver high-quality care to patients in a general practice setting.
  • Support health promotion activities across the PCN.
  • Run chronic disease clinics such as Hypertension Reviews.

Education

NVQ level 3 or 4 in Social and Healthcare
