Qualifications

• Registered with the GMC and on the GP Specialty Register.
• Post CCT experience in Primary or Urgent Care.
• Experience delivering care for patients with advanced illnesses.

Responsibilities

• Provide generalist and specialist palliative care across community settings.
• Lead anticipatory and advance care planning discussions.
• Support symptom management and reduce hospital admissions.
