Job summary

A well-known healthcare provider is seeking a dedicated Team Leader to join their award-winning team at Loxley Park Care Home in Sheffield. You will support the management team in delivering high-quality, person-centered care and ensure the well-being of residents. Ideal candidates will have an NVQ 3 in care and two years' experience in a senior position. A DBS check will be required, and the role is permanent with opportunities to grow in a caring environment.

We’re now looking for a warm, enthusiastic, and dedicated individual to join our award-winning team as a Team Leader at Loxley Park Care Home in Sheffield. If this sounds like the place for you, we’d love to hear from you!

ABOUT THE ROLE

Your focus as Team Leader will be to support the home management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

Other responsibilities will include:

  • Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents\' next of kin fully informed, where appropriate.
  • Leading with monitoring the well-being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
