About the Role

As a Community Nurse, you’ll play a vital role in supporting clients with a wide range of needs, including complex care, rehabilitation, and end‑of‑life support. Working closely with clients, families, and multidisciplinary teams, you’ll help create safe, personalised, and effective care plans that make every day better for those we support.

What You’ll Do
  • Deliver high‑quality, person‑centred nursing care
  • Complete assessments, care plan, and evaluate outcomes
  • Support clients with complex health needs in their homes
  • Administer medication safely and accurately
  • Offer guidance, support, and training to staff
  • Promote health, wellbeing, dignity, and independence
  • Maintain accurate clinical documentation
  • Contribute to continuous improvement and quality standards
What We’re Looking For
  • Registered Nurse (NMC PIN required)
  • Experience in community, acute, or complex care settings (all experience levels considered)
  • Strong communication and clinical decision‑making skills
  • Compassion, reliability, and a commitment to delivering outstanding care
  • Ability to work independently and as part of a supportive team
