Qualifications

• Degree in pharmacy required.
• GPhC registration as a Pharmacist necessary.
• Previous experience in clinical hospital pharmacy.

Responsibilities

• Conduct medication reviews for patients.
• Provide tailored treatment advice to patients.
• Support safe prescribing and medication reconciliation.
• Review and authorize prescriptions as per guidelines.
• Promote safe and cost-effective medicine use.
