Job summary

About the Role

Hassengate Medical Centre is looking to welcome a qualified and experienced clinical pharmacist into our busy practice, to play a full and active role within our multi‑disciplinary team. The role is offered on a full‑time basis of 37.5 hours a week.

The clinical pharmacist will work as an integral part of the general practice team within the Primary Care Network to optimise the use of medicines, improve patient outcomes, reduce waste, and support the management of long‑term conditions. They will provide clinical leadership on all aspects of medicines management within the practice and support safe, evidence‑based, and cost‑effective prescribing. The postholder will work closely with GPs, nurses, and other healthcare professionals to provide high‑quality, person‑centred care to patients, contributing to the overall efficiency and quality of clinical services.

Primary responsibilities include:

  • Conducting structured medication reviews and managing patients with complex polypharmacy.
  • Supporting management of chronic diseases and medicines optimisation.
  • Being an independent prescriber or be working towards becoming one.
  • Leading on medicines safety, compliance and antimicrobial stewardship.
  • Working closely with the multidisciplinary team and PCN colleagues.
  • Providing education and training to clinicians and staff on medicines use.
  • Contributing to practice and PCN service development projects.

About the Candidate

To be considered for this position, you will have the following qualification, skills, and experience:

Essential
  • Master’s Degree in Pharmacy (MPharm or equivalent)
  • Registration with the General Pharmaceutical Council (GPhC)
  • Enrolled in or completed the CPPE Primary Care Pharmacy Education Pathway (PCPEP)
