A busy healthcare practice in Stanford-le-Hope is seeking a qualified clinical pharmacist to join their multi‑disciplinary team. This full-time role involves optimising medicine use, leading on medicines management, and providing support to patients with long-term conditions. The ideal candidate will hold a Master’s degree in Pharmacy, be registered with the GPhC and possess strong communication and IT skills. The practice values patient-centred care and professional development, contributing to the overall quality of clinical services.
Hassengate Medical Centre is looking to welcome a qualified and experienced clinical pharmacist into our busy practice, to play a full and active role within our multi‑disciplinary team. The role is offered on a full‑time basis of 37.5 hours a week.
The clinical pharmacist will work as an integral part of the general practice team within the Primary Care Network to optimise the use of medicines, improve patient outcomes, reduce waste, and support the management of long‑term conditions. They will provide clinical leadership on all aspects of medicines management within the practice and support safe, evidence‑based, and cost‑effective prescribing. The postholder will work closely with GPs, nurses, and other healthcare professionals to provide high‑quality, person‑centred care to patients, contributing to the overall efficiency and quality of clinical services.
