About The Role

We are looking for a skilled, compassionate, and motivated Care Home Assistant Practitioner (CHAP) to join our dedicated night team. This role is ideal for an experienced healthcare professional who has completed accredited CHAP training and is committed to delivering safe, effective, and person‑centred care throughout the night. Working under the supervision of Registered Nurses, you will play an essential role in providing both clinical and personal support to our residents. You will carry out a variety of delegated clinical interventions, contribute to holistic care planning, and support the smooth running of the home during night hours. Your expertise will help ensure residents receive continuous, high‑quality care that promotes wellbeing, dignity, and independence.

Key Responsibilities
  • Support Registered Nurses in delivering exceptional and person‑centred care
  • Safely administer medication and undertake clinical procedures such as wound care, catheter care, and blood sugar monitoring (following competency sign‑off)
  • Assist with PEG/PEJ care, nursing observations, and other delegated clinical interventions
  • Record vital signs, food and fluid intake, and maintain clear, accurate, and confidential care records
  • Provide personal care and assist residents with mobility, repositioning, and activities of daily living
  • Promote resident dignity, independence, wellbeing, and choice at all times
  • Support, guide, and mentor care staff to enhance practice and maintain high‑quality standards
  • Contribute to developing, reviewing, and implementing care plans and risk assessments
  • Maintain a safe working environment and comply with all health & safety requirements
  • Uphold confidentiality, professionalism, and company values in every aspect of your work
About You
  • Accredited CHAP qualification (Care Home Assistant Practitioner training completed)
  • NVQ Level 3 in Health and Social Care, or an equivalent qualification
  • Minimum of 12 months' experience in a similar healthcare role
  • Competence in administering medication and delivering personal care
  • Experience working night shifts and comfortable with the responsibilities they involve
  • Good understanding of the Mental Capacity Act (MCA) and Deprivation of Liberty Safeguards (DoLS)
  • Ability to support residents with complex health needs, including tracheotomy and PEG/PEJ care
  • Strong communication skills, attention to detail, and a caring, professional attitude
