A long established client is currently looking for Experienced Advanced Clinical Practitioners to join a well-structured team due to growth of the service and an enhanced model of care. 40 hrs per week (full/part time available). The Advanced Clinical Practitioner will assess, diagnose and plan care for patients; including prescribing of medications where clinically indicated.

The ACP will manage acute presentations to the Walk-In Centre, as part of a team of autonomous Practitioners; in addition to using the clinical knowledge and skills to determine urgency and best pathway for patients presenting to the Walk-in Centre (streaming). Continuous professional development is key for our clinical workforce with the role having dedicated time for this and access to regular educational sessions. This is a hybrid role with opportunity to build an urgent care portfolio in a fast pace, dynamic service.

The full-time equivalent for us is 40 hours per week. Part-time can also be considered, conversations welcomed regarding flexible working.

Key Duties

1. Face to face consultations
2. Undertake urgent primary care and refer elsewhere, when applicable
3. Provide patient focused, evidence-based practice to a high standard
4. Supporting the wider MDT to ensure patients are seen by the right person

Key Skills, Qualifications and Client Requirements

1. First Level registered Nurse (NMC), HCPC professional/ Pharmacist
2. MSc or equivalent in Advanced practice
3. Advanced Physical Assessment and Consultation Skills
4. Level 7 A
5. Level 7 (desirable)
6. Ability to see and treat patients with varying conditions
7. Able to demonstrate excellent communication skills across all levels of the business
8. Excellent organisational skills
