About the role

To provide early and targeted support to aid the recovery and resilience of single adults or childless couples with multiple vulnerabilities, working flexibly across the city supporting people in their own homes within supported housing settings, or in locations of their choice.

Working closely with other agencies to provide seamless support to service users

Demonstrating a commitment to safeguarding in line with policy and procedure you will support vulnerable adults to develop the knowledge and skills they need to better manage their lives, including accessing appropriate services, empowering them to build successful relationships within their community, improving health and wellbeing and increasing levels of independence as they move through a prevention pathway model.

What you’ll be doing
  • Deliver a flexible service, increasing or reducing the frequency and intensity of support as required and re-introduce support to dormant cases should any tenancy or support issues arise.
  • Support customers to sustain accommodation, improve health and well‑being outcomes, develop skillsrequired to live independently, including offering practical support, maximising income, budgeting, addressing debt.
  • Act as a community navigator, linking customers into local services and activities to establish networks of support and reduce social isolation.
  • Co-ordinate interventions provided by other agencies to ensure that the right support is offered at the right time to enable the recipient to build resilience and prevent crisis or worsening health and well‑being.
  • Work inilarly with key agencies such as housing, health and substance misuse services and act as an advocate, unblocking barriers to facilitate access to services.
  • Lead on the completion of Risk Assessments with individuals and professionals, taking a proportionate approach to prevent or minimise risks using risk enablement techniques.
  • Demonstrate a personalised approach to safeguarding, working in line with policies and procedures and in accordance with the six key principles of safeguarding.
  • Support customers to access employment, education and training opportunities.
Who we’re looking for?
  • A positive attitude around working with people with multiple vulnerabilities.
  • Experience of agile working across locations to support a case load of customers is desirable but not essential.
  • Excellent communication skills
  • Proficient in using IT packages to capture all elements of support provided
  • Ablity to keep accurate and contractually compliant records
  • A relevant qualification specific to the role is desirable but not essential.
  • A working knowledge of legislation
  • Reliable and punctual to meet the needs of a diverse caseload of customers across different locations.
  • The ability to work both autonomously and as part of a team
  • Ability to maintain confidentiality and work in accordance with GDPR legislation
