Therapy Partner Reports To: Head of Therapy & Wellbeing

The Therapy Partner is the on‑site extension of the Constantia Healthcare Group Therapy & Wellbeing Hub. This role ensures that residents receive timely, appropriate and meaningful therapeutic care that enhances physical, cognitive, emotional and social wellbeing. Drawing on specialist expertise, the Therapy Partner delivers prescribed interventions, advocates for each resident’s therapeutic needs and ensures therapy principles are embedded into daily life. They contribute to functional improvement, independence, falls reduction, safety and quality of life. They work collaboratively with Home Operations Managers, Culture & Engagement Managers, clinical teams, NHS professionals and the wider CHG therapy network to ensure strong communication, accurate follow‑through and aligned decision‑making. Fundamentally, the Therapy Partner champions a find‑it‑and‑fix‑it approach, ensuring issues are identified early, addressed promptly and escalated appropriately.
