What you will do
  • Undertake Care Act assessments ensuring the highest standards of care and compliance.
  • Arrange and manage care packages when necessary, and continuously monitor their effectiveness.
  • Maintain ongoing monitoring of mental health, supporting individuals to remain well and thrive in their communities.
  • Collaborate as part of a multidisciplinary team (MDT), working closely with health colleagues to effectively manage risk.
  • Record and maintain comprehensive notes using the Paris system.
  • Purchase care packages and manage safeguarding procedures using Mosaic.
  • Ensure rigorous checking of references over a three-year period and verify gaps in employment history.
