Location: Barnsley Metropolitan, UK
Salary: £35.00 per hour
Contract Type: Locum, 6 Months
Hours: Flexible (Part-time & Full-time available)

We are looking for a Qualified Social Worker (QSW) to join the Adults LD & Autism Team in Barnsley Metropolitan. What You Will Do

Comprehensive Assessments: Conduct in-depth assessments of adults with learning disabilities and autism, documenting their social care needs.

Person-Centred Care Plans: Develop and implement individualised care plans that promote independence and community engagement.

Risk Management: Handle complex safeguarding issues and conduct risk assessments to protect vulnerable individuals.

Family & Professional Collaboration: Work alongside families, carers, and other professionals to create holistic multi-disciplinary support plans.

Monitoring and Reviewing Care Plans: Regularly assess and adjust care plans to ensure optimal outcomes for service users.

Record Keeping: Maintain accurate and up-to-date records that meet Social Care Practice Standards and council policies.

Case Conferences & Decision-Making: Participate in multi-agency meetings, share expertise, and coordinate support.

Mentorship: Provide guidance and mentorship to less experienced staff and social work trainees.

Mental Capacity Act Compliance: Support decision-making for individuals lacking capacity, ensuring compliance with the Mental Capacity Act 2005.
