Overview
The RN Clinical Manager coordinates and oversees all direct care patient services provided by clinical personnel. This role focuses on home health clinical quality assurance and home health clinical operations initiatives. Location: Liverpool, NY. Schedule: Full-time, 40 hours per week. Position type: Hybrid. Base pay range: $77,200.00/yr - $106,200.00/yr. Total compensation may vary based on experience.
Responsibilities
- Develop, plan, implement, analyze, and organize clinical operations for the location managed.
- Assess and reassess patients, update care plans, and interpret patient needs in accordance with company, physician, and facility procedures/policies.
- Manage the assignment of caregivers and oversee delivery of care to all patients served by the location.
- Guide clinicians to promote effective performance and high-quality home care services; provide availability during operating hours to assist clinicians as appropriate.
- Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing patient Plans of Care (POC).
- Monitor case documentation to ensure regulatory and third-party payer compliance; ensure timely audits and billing in compliance with Medicare regulations.
- Coordinate communication among team members, attending physicians, and caregivers to ensure appropriate care and outcome planning.
- Collaborate with Branch Director and Finance to establish location revenue and budget goals; participate in sales and marketing initiatives.
- Supervise all clinical employees assigned to the location; handle disciplinary actions in consultation with Human Resources and leadership.
- Participate in interviewing, hiring, and training of direct care clinicians; evaluate performance and coach staff; ensure adherence to internal policies and standards.
- Assess staff education needs, deliver regular staff education, and support professional development.
- Analyze situations, identify problems, and evaluate actions using Performance Improvement principles.
- Review case managers and clinical documentation for appropriateness of care, starts-of-care, resumption-of-care, and recertifications.
- Lead QA/PI activities; work with Utilization Review on data tracking to evaluate performance and outcomes; participate in care delivery and staff management as needed.
- Provide direct patient care only in emergencies; may act as Branch Director in absence of the director.
- Ensure compliance with Company standards and external regulatory requirements; interpret policies for staff and ensure documentation adequacy.
- Maintain relationships with referral sources and participate in professional organizations and care-related programs.
- Perform other related duties as assigned.
Qualifications
- Graduate of an accredited School of Nursing; BSN required (NY state requirement).
- Current state RN license and valid CPR certification.
- Proof of valid driver’s license, auto insurance, and reliable transportation.
- Two years of RN experience with at least one year of management experience in a home health environment.
- Certified Home Health experience required (NY state requirement) or four years in home care with related management education/training.
- Management and people leadership experience preferred.
- OASIS experience required; OASIS certification (HCS-O, COQS, COS-C) preferred.
- Homecare Homebase (HCHB) experience preferred; CMS PDGM billing knowledge/experience preferred.

Pay and Benefits
- Pay range: $77,200 - $106,200 per year; pay is a good-faith starting range and varies by location and experience.
