A leading healthcare organization in the UK is looking for a proactive Registered Nurse to provide exceptional palliative care and emotional support for patients in their home and other settings. Ideal candidates will have a valid NMC registration and experience in holistic palliative care. The role includes night shifts and will require flexibility to work various patterns across the area.

Main duties of the job
  • To provide ongoing assessment of patients needs during a shift of work for those patients with life limiting illness and those who are dying.
  • To assess patients condition and any deterioration that may have occurred, document and refer on to the appropriate personnel.
  • To talk with patients, explain, listen and advise them regarding symptom control (e.g. pain, nausea & vomiting, constipation).
  • To carry out direct nursing care (bladder & bowel management, nutritional management involving feeding regimes given via a PEG).
  • To assess the needs and administer medications either orally, by injection or by syringe driver as prescribed.

Hospice at Home's core service is our skilled team of Registered Nurses and Healthcare Assistants who provide exceptional care and support in people's last year of life, at end of life, also providing additional services such as Lymphoedema, Bereavement and Family Support, Befriending, Occupational Therapy and Complementary Therapy.

We provide individualised care and support to improve quality of life and our end of life care helps facilitate a peaceful and dignified death in the home and support to those who are bereaved.

Hospice at Home, an independent charitable organisation, is looking to recruit a registered nurse to join the nursing service. The post holder will be predominantly working nights across our geographical area. Providing support to patients that are palliative or end of life.

Job responsibilities

The post holder will provide direct nursing care predominantly in the patients home, and on occasions in other settings such as Community Hospitals, Residential & Nursing Homes and Eden Valley Hospice.

The post holder will provide nursing care, emotional, social & psychological support to individuals with palliative care needs, their families & carers. This involves regular and extended involvement of assessing fluctuating needs and the stress of caring for the dying patient and their families.

To take part in an on call rota covering hours outside standard service times.

Person Specification
Skills and Aptitudes
  • Enhanced communication skills to work with patients, families and health/social care professionals.
  • Ability to work in a complex environment, often in isolation and demonstrate effective coping mechanisms.
  • Quality focused with an innovative approach to solving complex problems.
  • Experience of electronic patient information systems e.g. EMIS.
Qualifications
  • 1st level Registered Nurse (Adult) NMC Registration.
Personal Circumstances
  • Flexibility to work various shifts patterns over a 24 hour period 7 days a week.
  • Must be able to travel autonomously across the geographical area covered by the service
Knowledge
  • Good knowledge of palliative care theory and practice
  • Knowledge of Palliative Care issues.
  • In-depth understanding of multi-disciplinary issues and commitment to inter-disciplinary working.
Experience
  • 2 years post registration experience.
  • Previous experience in delivery of holistic palliative/end of life care.
  • Previous experience within a hospice or a community based setting.
  • Experienced in providing supervision for others.
