About the Role – Physical Therapist

As a Physical Therapist at Elderwood, you’ll be a key member of our rehabilitation team, providing skilled, individualized therapy services to support resident mobility, strength and quality of life.

Responsibilities
  • Initiate skilled physical therapy treatments in accordance with physician’s orders; maintain required contact with physician.
  • Maintain written evaluations, progress notes and discharge summaries for individuals in the physical therapy program.
  • Confere with other health care staff regarding special needs of individuals to enhance rehabilitation or maintenance of function.
  • Provide input for meetings and attend during absences of the Director Physical Therapy.
  • Maintain statistical records of individuals’ treatments.
  • Assist with in‑service training programs upon request.
  • Record and update individual’s plan of care in the Inter‑disciplinary Kardex.
  • Submit requisition for supplies needed in treatment to Director Physical Therapy.
  • Assume all duties of the Director Physical Therapy during absences, including supervision of Physical Therapy Aides and responsibility for all necessary documentation.
Qualifications
  • Graduation from an approved school of physical therapy required.
  • State license to practice physical therapy required.
  • Two years’ experience in physical therapy preferred.
  • Ability to direct others as necessary.
  • Regular interaction with residents, coworkers, visitors and supervisors; ability to read, write, speak and understand English at an intermediate or higher level.
