Qualifications

Job description

To ensure high-quality lead support is given to each individual with a wide range of needs. You will be offering support with daily living and promoting independence where it is needed. This requires you to work closely with external agencies and your colleagues. You will be expected to adhere to all MYSHON policies and procedures whilst undertaking your duties.

Key Responsibilities

1. Work closely with individuals to identify needs and support with managing these.

2. Support individuals with daily living such as prompting medication, budgeting plans, attending appointments and engaging with external support services.

3. Monitor, identify and promote practice that safeguards individuals and follow the whistle blowing procedure to raise any concerns.

4. Listen and promptly deal with any issues and liaise with the housing manager where needed to support with this.

5. Liaise with appropriate external professionals in respect of tenants changing needs.

6. Inspect areas within the properties in order to arrange repairs and decorations and to ensure rooms are ready for new tenants to move in to. This includes promoting safe and clean use of communal areas and private rooms within the property.

7. Ensure relevant health and safety checks are carried out in accordance with the policies and procedures and report any issues.

8. Participate in team meetings, new initiatives and future changes to improve service delivery.

9. Work in a person-centred approach, with individuals at the forefront of what we do.

10. Ensure work practices meet Health & Safety requirements and adhere to the companies Equality and Diversity policy.

11. Respect confidentiality and data protection regulations (GDPR) at all times.

12. Work with the housing manager to provide advice and support to tenants with regard to neighbour disputes, harassment and any other housing related occupancy issues e.g. security – to include liaison with third party agencies where appropriate e.g. police, support staff, social services etc

13. Complete relevant paperwork which supports the tenants needs, reporting on these daily to monitor individuals.

14. To participate in training which is relevant to your role to meet the needs of individuals and to progress your skills and knowledge.

15. To monitor the security within the property and report any concerns.

16. To work in a professional manner at all times.

17. Other tasks that may be requested from time to time by your manager which are not in the Job Description, but which reflect the general nature and level of the job.

Experience and Qualification
  • Home Care: 1 year (required)
  • Working with Vulnerable Adults: 1 year (required)
  • Driving Licence (required)
  • Be willing to have an enhanced DBS check completed
  • Have good communication skills – verbal, written and listening
  • Have a good understanding of how to use Outlook, Word, and Excel
