Job Summary
Health Care Support Worker, Band 2, Permanent, 37.5 hours per week.
Working pattern 24‑hour shift on a rotational basis.
Midlands Partnership Foundation Trust provides comprehensive mental health services across Staffordshire and Shropshire. This role is based at Ellesmere House, a 12‑bed low‑secure learning disabilities / neurodiverse / MI service.
Job Responsibilities
- Communicate with service users, carers, multidisciplinary team, and external agencies using various methods including IT.
- Report and effectively communicate service users' health status and care aims in care records and verbal reports to Registered Professionals and other team members.
- Maintain professional boundaries and act in accordance with equality, diversity, rights and responsibilities.
- Identify, minimise and report risks of danger, harm and abuse to senior colleagues.
- Maintain a safe and secure environment for the protection of self and others.
- Employ safe working practices to minimise risks to self.
- Work within Trust policies and operational procedures.
- Coordinate with other Trust services to promote seamless care, admission, transfer and discharge along identified care pathways.
- Deliver planned therapeutic activities and interventions for a group of service users as part of the care programme approach.
- Help service users identify meaningful activities that support health outcomes.
- Support service users to access activities and services to maintain independence and promote recovery.
- Assist service users with personal care needs.
- Provide information, support and guidance to carers, friends and relatives.
- Engage in supervised development and provide support to colleagues, students, trainees and volunteers.
- Participate in activities focused on delivering high‑quality, customer‑focused service.
- Suggest improvements to service delivery.
- Take responsibility for supervision and mandatory training.
Person Specification
Essential Qualifications
- Level 2 Diploma in Health and Social Care OR
- NVQ 2 in Health and Social Care with demonstrable evidence of numeracy and literacy skills (equivalent GCSEs or Functional Skills) OR
- Demonstrable knowledge and experience as outlined below.

- Experience working as part of a team and delivering a customer‑focused service.
Essential Skills, Knowledge & Abilities
- Knowledge of mental health, learning disabilities, access to services and related health promotion, including inclusion issues.
- Literacy, written, verbal and ability to use electronic information and computer technology.
- Skill in managing high‑expressed emotions and ability to manage own emotional state.
- Ability to work independently, self‑motivated and to prioritise effectively under competing pressures.
