Female* Recovery Navigator (Wiltshire Service)

We have two opportunities available in our Swindon Team for Female* Recovery Navigators. You will join us on a Permanent Contract working 37.5 hours per week 8am - 6pm Monday to Friday on a rota basis, and in return, you will receive a competitive salary of £26,056 - £28,204 per annum plus benefits.

Key duties and responsibilities
  • Work as part of an integrative partnership to provide a joined-up offer to individuals accessing the service
  • You will liaise with colleagues and build a strong working partnership with other agencies such as GPs, Health and Social Care, other Voluntary and Community Services to coordinate referrals, take initiative to share learning new ideas and trends, and deliver the service user’s support package
  • Attend and contribute in a positive and proactive way to daily handovers and Multi-Disciplinary Discussions
  • Work with individuals through trusting relationships to collaboratively develop and review person - centred support plans that focus on their goals and hopes, using DIALOG+ support tool and evidence-based interventions
  • Working alongside AWP, PCLS, and other professionals to identify appropriate pathways of support to achieve these using Your team, Your conversation, Your Plan assessments and other self-management tools
Qualifications
  • Experience working with people with mental health needs, or people with complex needs
  • Proven knowledge of support needs of people with mental health needs
  • Proven experience of working with people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency long term conditions
  • A current, full driving licence and access to an appropriate vehicle, with business insurance is desirable
