Overview

Adult Community Learning Disability Nurse – Band 5. Full time; 37.5 hours per week (Monday to Friday, 9:00–17:00). Site: Bury. Salary £31,049 – £37,796 pro rata. Closing date: 20/02/2026 23:59. Interview date: 13/03/2026. Job ref 311-F892-26.

We are looking for a kind, fair, ingenious and determined nurse to join a multidisciplinary team in the Learning Disability Service. The role involves working unsupervised in the homes of service users and other community settings, collaborating with service users, families and carers to promote health and well-being and maximise independence. The post involves working with people who are vulnerable and may have communication difficulties or challenging behaviour.

Main duties of the job

Give a high standard of specialist nursing care to people referred to the learning disability team, and help them attain maximum independence and quality of life by assessing, developing, implementing and evaluating programmes of care and Health Action Plans in conjunction with service users and carers.

  • Offer support to families and service users to gain maximum benefit from available services and resources.
  • Take responsibility for the management of a caseload.
  • Undertake a range of assessments with service users and carers/families using a person-centred approach and identify and support Health Facilitators for people with learning disabilities in accordance with relevant strategy guidelines.
  • Provide a quality service in line with the service philosophy and the principles of the strategy for Learning Disability.
  • Contribute to risk management and critical incident analysis, and be involved in setting, monitoring, maintaining and auditing standards.

Detailed job description and main responsibilities

Main duties and responsibilities (condensed): see above for core nursing care, caseload management, person-centred assessments, and contribution to risk management and standards.

Patient care responsibilities include delivering teaching sessions, supporting people with challenging behaviours, developing strategies with carers and professionals, providing high quality community nursing for people with learning disabilities and complex needs, and working within a multidisciplinary team to achieve the best outcomes.

Education and professional development:

  • Engage with primary and secondary care to meet health needs
  • Attend study days and training; identify and pursue training needs
  • Educate service users, carers and others on Health Action Planning, Health Facilitation, Learning Disability Awareness and Challenging Behaviour
