We are looking for a kind, fair, ingenious and determined nurse to join a multidisciplinary team in the Learning Disability Service. The role involves working unsupervised in the homes of service users and other community settings, collaborating with service users, families and carers to promote health and well-being and maximise independence. The post involves working with people who are vulnerable and may have communication difficulties or challenging behaviour.
Give a high standard of specialist nursing care to people referred to the learning disability team, and help them attain maximum independence and quality of life by assessing, developing, implementing and evaluating programmes of care and Health Action Plans in conjunction with service users and carers.
We provide high quality mental health and learning disability services, both inpatient and community-based, across five boroughs of Greater Manchester (Bury, Oldham, Rochdale, Stockport and Tameside and Glossop).

Patient care responsibilities include delivering teaching sessions, supporting people with challenging behaviours, developing strategies with carers and professionals, providing high quality community nursing for people with learning disabilities and complex needs, and working within a multidisciplinary team to achieve the best outcomes.

The post holder must comply with professional standards and maintain relevant registrations. The Trust aims to maintain good working relationships with patients, staff and the public, and to deliver courteous, professional service at all times.
